I hereby certify that I am a volunteer participant in the “MUD” Volleyball Tournament; that I realize said program is being offered to me, knowing full well that I may attend and participate in all activities conducted by said program or not as I myself decide. I further certify that I hereby release and forever hold harmless from any claim, cause or suit, against the Lena- Winslow Jaycees and all sponsors or programs and assume all physical risks inherent therein. I understand the same. I have also read the forgoing release and waive to the team, and they do clearly understand the same. Team captain assumes all responsibility for players named and/or unnamed. Each team MUST supply one person from their team to referee the preceding game regardless if their team is out of the tournament.
